Brief Paperwork and History form
Upon entering the office, we have some paperwork for you to fill out, which provides us with general information about yourself and your condition.
Please bring in any diagnostic reports that have been done on you. You can get these reports from your Medical Doctor. Also you can request X-rays films from where they were taken by calling them ahead of time.
History and Examination
Various questions will be asked during the history taking followed by an examination where specialized tests are utilized to determine the nature and cause of your condition.
Explain your condition
Before a treatment is rendered, you are given the opportunity to have your condition explained to you in a thorough yet simplified, easy to understand manner. I find patients tend to get better faster when they understand what is happening with them.
If it is determined that your condition is beyond what this office can deal with, you will be referred in an appropriate manner.
Same Day Treatment
With your consent, a treatment is rendered during the initial visit that is appropriate for what you can handle in order to get “the ball rolling”.
Home Instructions
Prior to leaving, you will be given proper instruction on whether to use ice or heat as well as what type of stretching and strengthening exercise will best help you towards your recovery.
